In the front of medal you can see a drawing of a Hamsa with quotes from the Jewish sources and combinations of letters. A quotation from the Priestly Blessing: "יברכך ה' וישמרך, יאר ה' פניו אליך ויחנך, ישא ה' פניו אליך וישם לך שלום". On the margin the inscription "קמיע להצלחה ולשמירה מעין הרע מאדוננו האר"י הקדוש זיע"א, וברכת כהנים ממורנו ורבנו הרה"ג רבי רפאל כהן שליט"א".
In the back, there is a drawing of the community synagogue that was built in the year 2012 by the "Yated Teshuvah" institutions in Safed and around it the inscription"תבנית בית הכנסת הקהילתי שנבנה בשנת תשע"ב ע"י מוסדות 'יתד התשובה' בצפת, בברכה למחזיקי המוסדות שהוקמו בס"ד בשנת תשמ"ה".
